Providing guidance and support to enable the client to self-manage is critical to ensure that long term sustainability is achieved. Our programs include self- management techniques incorporating motivational interviewing, client engagement into program development and team planning /goal setting.
Clients may require 2-5 sessions depending on their current physical activity and function. This may be over a 6-12 month period or within a 12 week program designed to gain adherence, compliance and to motivate the more sedentary client. This approach allows for programs to be monitored, progressively increased and to ensure that physical health is at its optimum.
Once a client is successfully working through their program and meeting their personal goals, follow up appointments may only be required every 3 to 6 months.
At Functional Health Solutions we understand that when managing chronic disease this is often a long term approach that includes many short term success stories.
